LAS VEGAS TRIP PROMOTION TERMS & CONDITIONS*

To participate, all cases must be invoiced to your Studio 360 account within 6 months after the date first case is received. Your account must be paid in full and current according to Studio 360’s payment terms. For new customers only.

• The Las Vegas trip promotion is only redeemable by the customer/account holder and one guest. The trip is nontransferable to other individuals.


• The total value of the Las Vegas trip promotion for either reward level, may not be traded, bartered, or exchanged for Studio 360 account credit or total trip value in gift cards. Multiple or combination trip reward levels cannot be redeemed. Limit one reward level ($5,000 or $10,000) per customer.


• New customers / group practices with special price levels are NOT eligible for the Vegas promotion.


• Customers who have earned or redeemed one of the two reward levels toward the Studio 360 Las Vegas trip promotion, are not eligible to participate once again if their account is reactivated.

• “New customers only” defined as, customers who have never
had a previous account with Studio 360, “The Dental Lab.”

• Hotel reservation is the responsibility of the participant. A personal credit card is required for room deposit and incidentals when making hotel reservations.

• A minimum of two weeks notice will be required to redeem  your trip.  Flight Reservation is the responsibility of the participant.  A personal credit card is required for plane tickets when making flight reservation.  A maximum of $1,500 will be reimbursed by Studio 360 “The Dental Lab” to the participant for 2 round trip plane tickets.

• Visa gift cards in the amounts according to reward level redeemed will be given to you personally when you arrive in Las Vegas. Visa has no responsibility or liability for products or services provided by Studio 360 “The Dental Lab”.
